Emergency Form

Medical/Liability Release: In case of an emergency and I cannot be reached, I authorize the staff of Creative Kids, Youth Theater Project, Happy All Day Inc. to obtain whatever medical treatment deemed neccesary for the welfare of my child. I further understand and agree that I will be financially responsible for all charges and fees incurred in the rendering of said emergency treatment, regardless of whether or not my medical insurance would cover such charges and fees. I hereby give my consent for my child’s participation in the activities of Creative Kids, Youth Theater Project, Happy All Day Inc. and hereby absolve and release Creative Kids, Youth Theater Project, Happy All Day Inc. and it's teachers and representatives, from any and all liability for any injuries and damages that may be incurred by my child in the activities sponsored by Creative Kids. Having read and understood and being in agreement with this document, I have signed and dated this agreement
